I, the undersigned, do hereby submit my application for the West Island Karate Championship tournament held on the 13th of May, 2023, in Montreal. I hereby assume full responsibility for any and all injuries or damages or losses that may be sustained or incurred by me while competing or while in attendance at said event. I also acknowledge the inherent nature of this tournament and the possibility of injury as the competition involves contact. Therefore, I hereby waive all claims against West Island Karate / Karate de L’Ile or any person involved therein, or any agents of said organization, including but not limited to promoters, staff, judges, referees or competitors of the tournament against any injuries or damages or losses that may occur or that I may suffer, and hereby consent not to sue any such individuals or organizations involved in the said tournament for any reason whatsoever, as I hereby certify that I am assuming all risks resulting from this tournament. It is understood that this is a sporting event that involves physical contact. It is understood that any treatment given for any possible injury will be of first-aid type only. I give consent to any and all reproduction of my likeness created in any manner (pictures, videos and the like) for promotional or publicity use and waive any claim for compensation. Please note that this is an application and may be refused at the organizer's discretion, and that there will be no refund whatsoever by reason of any kind of disqualification or expulsion. The director reserves the right to verify any information by asking for picture identification or re-weighing of any competitor. Immediate disqualification without reimbursement from the tournament will be the result of any false information provided, including as to weight, rank or age. I have fully read, understand, and agree to all the terms of the application and rules of the tournament.

Par conséquent, dans le cas où il y aurait blessure, dommage ou perte, aucune plainte ne sera émise de ma part contre West Island Karaté / Karaté de L’Île ou toutes personnes impliquées, ou tous autres organismes ou agents, promoteurs, employés, juges, arbitres ou participants de ce tournoi, et je consens de ne poursuivre aucun individu ou organisation impliqué et donc j’assume tous les risques reliés dans ce tournoi. Il est convenu que ceci est un évènement sportif qui implique des contacts physiques et que tout traitement ou soins procuré, si blessure il y a, sera digne des premiers soins seulement. Je consens à ce que photos, vidéos ou autres, incluant ma personne, soient utilisés à des fins promotionnelles ou publicitaires. Je ne demanderai aucune compensation et ne porterai aucune plainte vis à vis celles-ci. Veuillez noter que ceci et une demande d’inscription qui peut être refusée à la discrétion de l’organisateur et qu’aucun remboursement ne sera émis pour aucune raison, que ce soit par disqualification ou expulsion. Le directeur se garde le droit de vérifier les informations fournies en demandant identification de photo ou en pesant le participant pour s’assurer de la validité du poids. Si quelconque information s’avère fausse, que ce soit au niveau du poids, du rang ou de l’âge, il y aura disqualification immédiate. J’ai lu, compris et accepte les règlements qui s’appliquent à cette inscription et au tournoi.
